What is Support Coordination?
Support coordination is a service that can help you make the most of your NDIS plan by building your capacity and enabling you to achieve your goals.
Our Support Coordinators will help you to:
- Understand and effectively use your NDIS plan.
- Connect you with NDIS providers, as well as mainstream, community, and other government services.
- Develop your confidence and skills to manage and coordinate your supports.
You will learn how to best use your NDIS funding to maximise your capacity for growth and reach your goals.
Your support coordinator will get to know you as a person and help you choose who you would like in your life to aid you and which activities you would like to explore. In the challenging times that we face in the Kimberley with limited resources, your support coordinator can also help through moments of crisis and guide you and connect you to medical entities and other intensive supports.
Support Coordination – Level 2
We support participants to build capacity in all areas of life and to access their local community. We also help connect service providers and monitor NDIS Plan outcomes efficiently.
Specialist Support Coordination
We provide participants with support for more complex challenges during each step of their NDIS journey. We also assist during times of respite or transitional periods.
Psychosocial Recovery Coaching
We use our specialist skills and knowledge of mental health to navigate the NDIS and provide mental health specific supports.
